    Job Description:
    To facilitate timely processing of ED patients and their medical records using multiple software applications. The position manages information and connects physicians, staff, and visitors with appropriate resources.

    ADDITIONAL REQUIREMENTS
    Use of usual and customary equipment used to perform essential functions of the position.
    Work may occasionally require travel to other Carle Health facilities/hospitals.
    Required to drive your own vehicle for business purposes.

     
    SKILLS AND KNOWLEDGE
    Writes, reads, comprehends, and speaks fluent English.
    Basic computer knowledge using word processing, spreadsheet, email, and web browser.

    Critical thinking skills using independent judgement in making decisions.


    Essential Functions:
    • Retrieves records after discharge and performs general office duties.
    • Retrieves all records of discharged patients from all units (including inpatient, outpatient surgery, emergency department, etc.) as assigned.
    • Checks daily discharge logs to assure receipt of all records.
    • Reports to Manager when unable to obtain a patient record within 24 hours post discharge.
    • Answer the main department phone line within 3 rings. Uses the automatic caller distribution (ACD) system appropriately. Uses telephone etiquette consistent with Methodist Begins with Me principles.
    • Completes requests for health information according to policy and procedure.
    • Cleans copier/fax machines and ensure they are in proper working order and place service calls as appropriate.
    • Keeps workstations and work area clean and orderly.
    • Identifies processing flow issues and works to resolve or report to supervisor.
    • Performs other duties as assigned.
    • Prepares and scans records to be scanned according to procedure.
    • Checks each medical record for poor originals, photographs and other difficult to scan documents according to procedure.
    • Ensures patient name and encounter number are documented on each document to be scanned.
    • Prepares all documents by removing staples, paper clips, sticky notes, and rubber bands.
    • Groups like document types together, place in page order and in date order.
    • Copies all damaged documents so that they can go through the scanning process without difficulty.
    • Removes COLD fed documents as defined from each record.
    • Securely attaches all rhythm strips and other small documents to 8 1/2 X 11 paper for scanning.
    • Assembles all documents in appropriate order as outlined in chart prep policy.
    • Retrieves loose reports from the loose reports inbox daily and prepare for scanning.
    • Prepares batches by placing an appropriately filled out batch cover sheet on each batch and placing rubber band around batch.
    • Ensures entire record is ready for scanning.
    • Completes scanning process according to procedure.
    • Maintains appropriate set up for scanner to ensure optimal image results.
    • Verifies each batch has been prepped appropriately.
    • Assigns a batch label according to procedure.
    • Write batch to correct work queue.
    • Cleans and calibrates scanners on a daily basis as indicated and maintains log.
    • Performs charging and data entry functions as related to patient's medical record.
    • Accurately and timely enters of all unit generated patient charges.
    • Reviews medical record for the accuracy and recovery of any missed charges
    • Assigns Visit level charges (E/M codes) to each account per ACEP guidelines.
    • Performs disposition and physician data entry into hospital software application (Star)
    • Performs clerical and other duties as requested efficiently and accurately to facilitate quality
      patient care
    • Performs required clerical duties accurately.
    • Displays knowledge of location and applicable procedures of various departments to run errands.
    • Demonstrates knowledge of most common requests made.
    • Demonstrates knowledge of ordering supplies for the unit as delegated.
    • Maintains patient charts in order.
    • Maintains work area in an organized manner.
    • Performs duties necessary for admitting, transferring, and discharging patients.
    • Accepts direction from lead RN to facilitate patient care.
    • Runs errands and performs extra duties as requested.
    • Able to change priorities as unit climate changes.
    • Uses slow time productively.
    • Unit involvement.
    • Willingness to assist peers in assignment completion.
    • Maintains open communication.
    • Assists in achieving department goals and objectives.
