Clinical Facilities Planning Manager jobs in Utah

Looking for the right fit to join our growing team at Utah Senior Planning. Utah Senior Planning is a financial firm that helps the elderly apply for government programs. We are contracted with nursing facilities throughout Utah to help them with all of their residents.  

Job Responsibilities

• Facilitate the Medicaid process for residents in a Nursing Facility
• Provide Weekly updates to contracted Nursing Facilities
• Schedule and meet with all potential residents that will require Medicaid 
• Responsible for sending and tracking communications for Medicaid enrollees. This includes following up with submitted recertification and submissions to Department of Workforce Services.
• Ensure punctual review of Notice of Action letters for enrollees to include obtaining any requested documents.
• Continual communication and follow up with residents in preparation of obtaining document request from DWS to maintain eligibility.
• Assist enrollee or designated representative in completion of Medicaid application, recertification's or applicable assistance program.
• Monitor and advise enrollees on Medicaid process and procedures
• Provide enrollees and designated representatives with information on re-certification dates and or benefit related questions.
• Visit with contracted Nursing Facilities weekly
• Manage 30 cases per month
• Other non -essential duties assigned.

Requirements

• Bachelors Degree (preferred but open to the right individual)
