A 250 bed facility with over 100 years of service to their community is searching for a Case Manager. The Case Manager will have the opportunity to coordinate a patient-focused team while utilizing the understanding of all aspects of nursing. Reporting to the Manager of Case Management, the Case Manager will support and assess the needs of the patient and family, set priorities for their needs, and monitor clinical status.

The ideal candidate obtaining a quick interview for this position will hold an active RN license, BSN, and BLS certification. A minimum of 4-5 years nursing experience and basic knowledge of evidence-based criteria, discharge, planning, care coordination, and community resources. An excellent opportunity for someone looking to care for patients in an outstanding environment. Critical thinking skills, excellent verbal and written communications skills, and a certification in specialty is a plus.
