DUTIES
Direct the coordination of client care and caregivers under the supervision of the Client Care Manager / Administrator
RESPONSIBILITIES
Perform on site assessments and quality assurance visits to clients’ home to assure complete satisfaction of care services.
Communicate on a routine basis with contract agencies, clients, clients’ families to maintain smooth relations and to obtain needed information.
Handle matters related to staff and the clients they serve. Including but not limited to, representing our services in a professional and creative manner.
Document, report, review and direct case management notes regarding change in schedule, change of caregiver, change in service, telephone complaints, and/or other situations that arise with staff and clients, etc.
Supervise the scheduling of appropriately skilled caregivers to fill shifts. Manage the input of the scheduling information on the computer and continue to supervise the coordination of the scheduling.
Supervise the scheduling of replacement caregivers as needed and the input of the scheduling information. If unable to find coverage you are to cover the shift(s).
Supervise and monitoring of tasks done by the Lead caregivers. If the leads are unavailable, you are to perform their duties and responsibilities.
Assist in interviews and hiring of care givers and set-up of personnel files.
Supervise field staff in regard to punctuality, reliability, assignments and related matters.
Prepare written performance evaluations of field staff.
Discipline field staff to include making recommendations for termination. This may include counseling and making recommendations regarding suggested discipline, or corrective action where performance problems exist.
Provide a daily report to the client care manager supervisor.
Leading/ Assisting with orientations, in services trainings, and other company programs or initiatives to enrich the quality of our care giving staff.
Assist in time adjustments and/or other needed task for payroll and billing as directed by client care manager supervisor as needed.
Answering calls from referral sources, prospective clients, and/or their family members. Offering education to them about out services and care options.
Performing initial assessments, assist in applying for Medicaid waiver or Medicaid. Conducting a following up on initial assessment(s).
Maintaining accurate records on all prospective, and active clients, employees, and referral sources
Visiting and arranging local meetings/appointments with potential clients/referral sources, and developing new business relationship(s) and referral networks as needed.
Performance of other duties as required.
JOB CONDITIONS
Position is stressful in terms of meeting deadlines.
It is a mix of field job and desk job which essentially involves sitting, standing, stooping, and walking, as well as an inordinate amount of telephone communication. On the field which requires driving, providing client care(s), and assistance to caregivers. You might also be out and about marketing and intaking new clients.
It requires minimal lifting of office records and printouts.
It requires continuous open communication with the Client Care Manager Supervisor/, and the Administrator about clients and caregivers.
One must be able to hear adequately on the telephone with no more than an amplifier and be able to communicate both verbally and in writing in English.
Requirements:
1. A valid driver’s license
2. Work schedule requires at least 1 weekend.
3. Must pass the company background check.
4. Must take an 8 hour IHSS Provider Training for In Home Support Services.
