  • Client Care Coordinator will serve clients across a variety of settings. Will work with patients in hospitals, shelters, assisted living facilities, clinics, group homes, and nonprofit organizations. Focuses on actively listening to clients and their caregivers, friends, and families. Client Care Coordinator will serve as an effective problem solver. They assess clients and their situations to make positive changes. Must maintain knowledge of current programs and services to offer practical and timely solutions. Creativity is key, because some cases can demand problem solving that involves multiple generations and differing age-specific needs. Will oversee several clients at a time, ensuring information is accurate, maintaining and keeping detailed documentation of each client. Engaging with clients from diverse backgrounds requires this position to have developed sensitivity to, and a general understanding of, a variety of cultures, religions, and social norms.

    Duties & Responsibilities

    • Daily check in with clients to identify and follow up on needs
    • Maintaining communication with hospital staff
    • Documenting all interactions with clients through shared excel spreadsheet
    • Help and support patients and family for care coordination by creating, assessing, and evaluating care plan processes and concerns
    • Collaborate and interact with other institutions
    • Analyze and evaluate residents upon admission and ongoing
    • Evaluate patients and identify the sort of assistance they require
    • Work with the Director of Client Services to develop treatment programs
    • Research and arrange community services
    • Provide transport via company vehicle/van for patients to appointments
    • Schedule shifts by matching the needs of our clients with caregiver skills, experience, availability, and personality
    • Track client and employee satisfaction and resolve any complaints under direction of Director of Client Services
    • Meet with clients and families to assess care needs
    • Introduce caregivers to clients
    • Conduct regular home visits
    • Rotating on call assignment

    Requirements

    • High School Diploma or equivalent
    • Home care experience: 1 year (preferred)
    • Health Care Experience: 1 Year (preferred)
    • Case Management experience: 1 year (preferred)
    • Scheduling experience: 1 year (preferred)
