Population We Serve
Children 3-17 years old with a behavioral health diagnosis or symptoms.
Parents of the children with a behavioral health diagnosis or symptoms.
This position may also be working with Adults and Seniors with disabilities at our partner agency: Adult Community Services

Flexible Schedule (Mon.-Fri.)
Position is offered Internship, part-time, full-time (32-40 hrs a week). Hours may fluctuate due to client cancellations.
Part-time position require a min. to accept 10 clients.
Position hours increase weekly in the first month as you build your caseload and make your schedule.
Current caseload is approx. 20-25 children, families, adults, and seniors for full-time
Position will require travel to service clients homes.
Pay with Benefits $22-$26/per hour plus quarterly bonuses

Commission Plan:
10-19 billable hrs/wk = 1hr of your pay
20-29 billable hrs/wk = 2hrs of your pay
30-39 billable hrs/wk = 3hrs of your pay
40hrs billable hrs/wk = 4hrs of your pay
Example:
10-19 billable hrs/wk = $22
20-29 billable hrs/wk = $44
30-39 billable hrs/wk = $66
40hrs billable hrs/wk = $88
Annual Commission (based on 40hrs/wk) = $4,576.00

Education:
Bachelor’s degree in social work, counseling, psychology or a related health care field, from an accredited institution.
Training & Experience:
At least 3 years working with at risk children, teens, and families.
General Description of Duties:
Provides behavioral health services to children, adolescents, and their parents. Adheres to the spirit of the mission while performing assigned duties.
Position will require to work outside of a 9am-5pm schedule at times based on family’s schedules.
Provide Life Skills services (assist with applications, paperwork, IADL’s) to Medicaid children, youth, parents, adults, and seniors.
Document and tracked outcome measures through Life Skills assessments, service plans, regular documentation through progress notes, and complete billing.
Comprehensive assessment/periodic re-assessment of the individual’s need for medical, educational, social or other services.
Activities/Interventions to gather/confirm information coming from the Individual, family and other sources in order to complete assessment.
Determining with the individual /family /supports their ability to access and effectively link to these services and supports on their own and what type of help they will need, including how intensely and for how long support services will be needed.
Supervision Received:
Works under the direct supervision of the Life Skills Team Lead.
Knowledge, Skills, and Abilities:
Ability to collaborate and work as a team with therapists in the treatment of psycho-social issues.
Sensitivity to low income and ethnic minority communities a must.
Experience working with Medicaid populations.
Technical skills:
Must be able to work on Excel, Microsoft word, and Adobe to complete required paperwork.
