Job Title: Housing Support Case Manager (LADC/ADC-T)

Supervisor: Clinical Supervisor

Salary: $28-31 hourly

Status: Full Time, Non- Exempt

Job Purpose: To provide case management services to Native American and other underserved populations to improve the safety, dignity and general quality of life.

Qualifications:

• Must have an alcohol and drug counseling license issued by the State of Minnesota and a Bachelor Degree in Human Services or a related field.
• Experience working with adults who have substance use disorders is preferred.
• Strong written and verbal communication skills
• Excellent computer skills, the ability to navigate electronic health records
• Excellent time management, organization and flexibility.
• Must be skilled in handling crisis situations.
• Must be able to pass background check by DHS and a national background check.
• Must have a valid Minnesota driver’s license and proof of insurance.
• If applicant has a history of chemical dependency, he or she must be in compliance with DHS 245 G. Two years of confirmed abstinence from all mood altering substances.

Duties and Responsibilities:

• Provide case management/file documentation for individual caseload, noting all treatment related activities and services in an accurate, professional, and timely manner.
• Develop, maintain, and record individualized client treatment plans, progress notes, and demographic information.
• Maintain a complete and updated case file for each assigned client.
• Adhere to all licensing, regulatory and agency guidelines.
• Provide job search and resume preparation when needed.
• Facilitate appointments for healthcare, dental care, and substance use disorder treatment.
• Provide case coordination for client case load.
• Prepare and submit required paperwork to counties.
• Create chore list and conduct room and house checks houses.
• Participate in a multi-disciplinary team in providing treatment for clients.
• Communicate with and coordinate services with other treatment providers when appropriate.
• Handle additional responsibilities, as assigned, for all clients; and to assume all other duties as assigned by the Clinical Supervisor and/or Executive Director.

Job Type: Full-time

Pay: $28.00 - $31.00 per hour

Expected hours: 40 per week
