As the Chemical Dependency Therapist, you will: •Conduct substance abuse assessments, conduct individual, group and family therapy sessions, and complete the discharge process. •Provide psycho-educational groups, 1:1 counseling, family education and assists patients with developing a recovery management plan. •Attend interdisciplinary treatment team meetings to propose and develop strategies and interventions that would promote excellent therapeutic outcomes. •Coordinate with the interdisciplinary team to provide referrals and community resources for aftercare and supportive services to ensure a successful discharge planning process. Candidate will promote positive patient-family-community relations.
