The Office for Children with Special Health Care Needs provides comprehensive services for children and youth with special health care needs through our pediatric specialty clinics, care coordination, case management, therapies, and collaborations.

Coordinates the Provision of Clinical Services to Children with Special Health Care Needs and Serves as Advisor and Resource Person to Professional Staff in the Prestonsburg section.
Key Responsibilities:
- Provides Care Coordinator services and direct care for caseloads of Children with Special Health Care Needs. Monitors progress to their physical and behavioral outcomes. Implements patient assessments. Specialized Nursing care of patients which may include care plan development, quality assurance and makes recommendations to the Nurse Administrator.
- Plans supportive actions to promote optimum health using family centered model of care. Ongoing evaluation of the plan of care. Follows up on patient needs and outcomes. Collaborates with personnel and team to meet patient objectives.
- Adheres to quality standards. Audits records. Advocates for patients and their families while coordinating effective health care services. Facilitates transitions appropriately.
- Conducts Outreach and attends meetings as appropriate. Provides health teaching to patients and families. Develops patient education programs and counsels’ families.
- Collaborates with patients, their families and others to maximize patient resources and facilitate decision making process. Participates as an interdisciplinary team member
- Maintains compliance with personnel requirements and follows personnel guidelines. Notifies managers and Human Resource Liaison appropriately on human resource related issues.
- Performs Other Duties as Assigned.
