General Summary
Provides spiritual counseling and guidance to patients, families and staff and performs or arranges for religious services to meet patient needs.
Duties and Responsibilities
Essential Functions:
- Participates in a spiritual assessment of the patient/family/caregivers, as indicated.
- Facilitates the development of a plan of care to meet the identified spiritual needs in conjunction with the interdisciplinary team.
- Provides direct spiritual care to patients/families/caregivers/staff, when needed.
- Serves as liaison to community chaplains and spiritual counselors, as indicated.
- Documents all patient/family/caregiver contacts and provides additional written, pertinent information as indicated.
- Participates in patient care conference and interdisciplinary team meetings.
- Provides consultation, education and support to the interdisciplinary team on spiritual care.
- Provides for funeral or memorial services or other services as requested by patient/families.
- Provides bereavement support.
- Provides spiritual support to staff.
- If Assigned to WellSpan Ephrata Community Hospital or WellSpan Good Samaritan Hospital: - Onboards and orients PRN chaplains to hospital logistics, key employees and location. - Mentors PRN chaplains in hospital processes and communication. - Keeps PRN chaplains updated and informed about changes to the hospital processes and logistics. - Coordinates the work of the PRN chaplains on the units and in the hospital. - Works out on-call schedule with PRN chaplains. - Meets regularly for coordination of working together.
Common Expectations:
- Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s).
- Enhances professional growth and development through participation in educational programs, current literature, inservice meetings, and workshops.
- Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.

Qualifications
Minimum Education:
- Bachelors Degree Required
Work Experience:
- 2 years Relevant experience. Required
- Hospice/home health experience. Preferred
Driver's License Statement:
Must possess a valid driver's license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.
Courses and Training:
- Graduate of accredited seminary or school of theology Upon Hire Required and
- Ordained/Endorsed by recognized religious body/proper ecclesiastical authority. Upon Hire Required
- Certified (or eligible) Chaplain by Association of Professional Chaplains (APC). Upon Hire Preferred

- Ability to network with community clergy and congregations.
- Knowledge of and commitment to Hospice philosophy of care.
