Our team of professionals, including a medical director, nurses, hospice aides, social workers, and volunteers, work in coordination with the patient’s physician to provide pain and symptom management, as well as social and spiritual support.

    Summary

    Responsible for providing pastoral care services and spiritual support to terminally ill patients, their families and community bereaved. Serves as an active member of the interdisciplinary team. Serves as a liaison with community clergy.

    Responsibilities

    Provides pastoral care and spiritual support to the terminally ill and their families.

    · Provides direct spiritual care to patients and families/caregivers.

    · Assesses for spiritual and religious strengths/needs.

    · Demonstrates an in-depth and broad understanding of spirituality and how spiritual and religious strengths/needs can be assessed and supported or addressed.

    · Develops the spiritual plan of care reflecting the assessed spiritual and religious strengths/needs.

    · Collaborates effectively with patients, caregivers, and the health care team to develop and implement an individualized spiritual plan of care.

    · Utilizes pastoral interventions to address spiritual needs and support spiritual strengths.

    · Demonstrates a wide range of spiritual care skills to discern and address complex spiritual and religious needs of patients/care-givers.

    · Conducts ongoing evaluation of the effectiveness of the spiritual plan of care.

    · Documents all pastoral care interventions and outcomes accurately and in a timely manner.

    · Demonstrates effective communication with patients, caregivers, and team members.

    · Attends patient care conferences and interdisciplinary team (IDT) meetings as a member of the interdisciplinary team.

    · Manages unexpected and/or unplanned events utilizing appropriate internal and external resources.

    · Reconciles personal spirituality with the varied needs and beliefs of others.

    · Recognizes ethical considerations in end-of-life care.

    · Provides and/or facilitates religious services or rituals.

    · Participates in providing pastoral care after hours, weekends, and holidays, as directed by the needs of the agency to provide coverage on a 24-hour basis.

    Provides pastoral care and spiritual support to staff, and the community, including the church communities in the areas we serve.

    · Provides consultation, education and support to the interdisciplinary team on spiritual care.

    · Serves as liaison and support to community including chaplains, ministers, spiritual counselors and funeral directors.

    · Actively participates in developing relationships with the spiritual community, including providing programs and services related to end-of-life and grief.

    · Plans/participates in periodic memorial services to meet the needs of staff, volunteers, and community clergy/spiritual counselors working with the hospice team.

    · Participates in the planning and provision of bereavement support groups.

    · Actively participates in the bereavement program, including counseling, when indicated.

    · Participates in other community programs as a representative of the agency, as needed.

    · Maintains ongoing professional development in the pastoral care field, particularly as it relates to end-of-life care and bereavement; shares their knowledge with appropriate organization personnel.

    Education and Experience

    · Master of Divinity or equivalent.

    · 2 Units of Clinical Pastoral Education (CPE) preferred.

    · Letter of endorsement and good standing with a recognized faith community.

    · One (1) year of experience as a minister or chaplain.

    · Prefer hospice experience.

    · Demonstrated ability to communicate effectively.

    · Basic computer skills.

    Licenses and Certification

    · Current appropriate licensure in the State of Kansas.

    · Valid drivers’ license.

    · Proof of current auto insurance.
