We're looking to hire a part time staff nurse with a track record of reliability and a can-do attitude. The right candidate for this position will be compassionate and have strong communication skills. This position is responsible for providing skilled in home care to all patients assigned to their case load.
Responsibilities may include but are not limited to: conducting comprehensive assessments of patient's chronic conditions, providing services as ordered by the treating physician, educating patients and their families on disease process management, and assisting the Case Manager with care coordination as needed, such as DME/HME. This position will also be responsible for providing accurate and timely documentation of the services provided during their shift and any observations of new/worsening symptoms or progress toward goals.
Qualifications:
- Must have reliable transportation
- Ability to read and follow written instructions in regard to the established plan of care
- Ability to work both independently and with a team, sometimes having minimal supervision
- Compassionate and patient-centered approach to care
Requirements:
- Must possess a valid RN license in good standing with the state board of nursing
- Minimum of 1 year of experience in home health or related healthcare field
- Must be able to pass a background check and drug screen
- Must have reliable transportation and valid insurance
