We are looking for compassionate and energetic RN Case manager who is passionate about providing high quality nursing services in a home health setting. Someone who has experience in comprehensive health and psycho-social evaluation, start of care, ROC, Recert, D/Cs, and can provide skilled nursing care according to client's plan of care.

Job will be a mainly in the office with nursing visits as scheduled. Our offices are located in 75023. Please note that this is not a work from home job.

Job Requirements:

• Conduct comprehensive clinical assessments of patients: SOC, ROC, Recerts, D/Cs, and Skilled Nursing visits to define Plan of Care (POC).
• Develop and manage client's plan of care
• Responsible for overall clinical records and documentation in the EMR solution
• Supervise all LVN visits and review and approve their charting.
• Implement individualized POC and recommend revisions to the plan as necessary.
• Initiating appropriate preventive and rehabilitative nursing procedures.
• Preparing clinical and progress notes that demonstrate progress towards established treatment goals.
• Coordinate all patient care and prioritization of needs with interdisciplinary team.
• Use of case management approach and referring to other services as needed.
• Inform physicians and other personnel of changes in the patient’s health & required needs and outcomes of intervention.
• Determine scope and frequency of services needed based on acuity and patient needs.
• Conduct weekly clinical quality review with Director of Nursing regarding patient care.

Position Qualifications:

• Registered Nurse (RN) with current licensure to practice professional nursing in Texas.
• Thorough hands on knowledge of core nursing skills in a Home Health Setting.
• Ability to apply the knowledge of nursing practices, techniques, procedures, and principles to provide quality nursing care to our patients.
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
• 1-3 years of experience in a Home Health Setting
• Experience in Home EMR systems like Kantime or Home Care Home Base or Kinsser
• Reliable car and auto insurance
• Clean background check and criminal record
• Understanding of regulatory compliance with state and federal regulations
• Will adhere to HIPAA and maintain client confidentiality
• Experience with OASIS and Medicare coding is a plus

Job Type: Full-time

Pay: $45.00 - $55.00 per hour

Expected hours: 30 – 40 per week
