Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature.     1 , Shift: Day, Schedule: 8am-4:30pm Monday-Friday

    Position Summary

    The Mental Health Specialist Community (MHS) functions as part of a collaborative, interdisciplinary treatment team responsible for providing outreach and assistance to persons with serious mental illness who are homeless or at imminent risk of becoming homeless. As a Mental Health Specialist you will assist clients in successfully transitioning from homelessness; secure funding, food, transportation, medical services, and other social services as needed and connect and refer clients to community mental health and substance use treatment centers.

    Requirements

    • Bachelor Degree required; preferably in human service field such as Social Work, Counseling, Psychology, etc; Certification as a Level 2 Psychiatric Technician through the American Association of Psychiatric Technicians (AAPT) may be accepted in lieu of the degree, at the discretion of the hiring manager and Human Potential
    • Must be eligible and apply for a Washington State Agency Affiliated Counselor within 30 days and obtain within 90 days of placement in job, unless a higher credential is held
    • One (1) year of mental health direct care experience required; experience in inpatient setting preferred
    • Basic Life Support Health Care Provider card through American Heart Association

    Pay and Benefit Expectations

    We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $21.05 - $30.29 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
