Case Manager, Sr. jobs in Rhode Island

The East Bay Community Action Program (EBCAP) is seeking a full-time Case Manager/Smart Optimal Solutions (SOS) Coach to support the LIHEAP (Low Income Home Energy Assistance) program located at 100 Bullocks Point Avenue in East Providence, RI.

• Provide one on one assessment to clients of the LIHEAP Program, who are seeking case management and further assistance, including long term monitoring of clients, providing energy conservation information, financial education, and refer to workforce development as needed.
• Attend community events to extend services through other social service agencies and community partners.
• Maintain relationships with collaborative partners and cultivate new ones. Responsible for collecting and analyzing data for LIHEAP and reporting on Smart Optimal Solutions Program.
• Develop and provide workshops and work with clients to set goals to complete identified program tiers.

Qualifications:

• High School/GED with equivalent experience required, Bachelor’s Degree in Human Services or related field preferred.
• Ability to provide high quality, strength-based, culturally sensitive assessment of consumers.
• Knowledge of state and community services/resources and ability to be proficient in securing those services for each consumer taking into consideration their individual needs
• Ability to travel between local EBCAP & community sites.
