Case Manager, Sr. jobs in Delaware
Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature. The Case Manager provides guidance and support to students who have gained employement
ESSENTIAL DUTIES AND RESPONSIBILITIES:- Develop and maintain a list of local service providers with which to leverage free and/or reduced co services for program participants.
- Monitor and evaluate effectiveness of individual training programs, maintaining related records of program activities and prepare reports detailing implementation and program results.
- Provide retention services to program participants-services include informal counseling, mentoring support, assistance with supportive services as needed.
- Develop and maintain an extensive list of local service providers with which to leverage free and/or reduced co services for program participants.
- Maintain frequent contact with participants to ascertain program performance and special needs.
- Give feedback on performance issues related to participants and actual retention solutions provided.
- Manage retention reporting, including obtaining necessary paperwork, pursuant to the contract reporting requirements.
- Perform other duties as assigned.
QUALIFICATIONS:- Minimum Bachelor’s Degree in Human Resources Development, Social Work, Social Services, Counseling, Education or related field.
- Working knowledge of workforce development.
- Working knowledge of community resources and various benefits/entitlements to support low-wage workers and small/medium-sized employers.
- Minimum of 5 years training development and delivery preferred.
- Excellent written and oral communication skills.
- Ability to engage adult learners in training activities.
- Proficient in Microsoft Word, Excel, Access and PowerPoint.
- Willingness and ability to work as a member of a team.
