We are seeking a full-time (40 hours per week) Wound Care Nurse Practitioner for float position between our Tygart Center and Pierpont Center, both located in Fairmont, WV. This role cannot support a recent graduate.
Position Highlights
- Evaluate patient health status, diagnosis, interpret labs and diagnostics
- Develop and implement patient treatment plans in collaboration with interdisciplinary care planning process
- Conduct weekly wound rounds and document in GehriMed according to GPS policy
*Assess, manage and treat wounds, identify wound etiology and cause of delayed/non-healing wounds*Address quality of life concerns related to wounds and actively develop and implement curative,
maintenance, or palliative wound care goals to meet the patient/family goals of care, incorporating
advanced care planning *Complete Genesis wound orientation, including Review of Center Framework for skin Integrity and approved Genesis Formulary products
- Serve as a clinical expert in wound care for nursing staff and provides targeted clinical education

Requirements: *Must be a graduate from an accredited School of Nursing program, licensed as a registered nurse, certified as a Nurse Practitioner *Graduate from an accredited Nurse Practitioner Program with a Master's Degree in Nursing is required. Concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP) is preferred. *Wound Certification: CWS, WOCN, or appropriate wound certification, or actively pursuing wound certification with APN wound experience. *A minimum of two years' experience in wound care, working with the adult or geriatric population is preferred. *In states where practice allows; APRN with an unrestricted professional license and DEA, for each state of practice.
Compensation: $108000 - $116000 / year
