Case Manager - Physician Practice coordinates the overall interdisciplinary plan of care for assigned patients in a physician practice setting. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided and that patient is making progress towards treatment goals. Position Summary:
The Practice Manager is responsible for providing feedback and assisting in the implementation of goals, objectives, policies, procedures, budgets, performance standards, and systems for physician practices. Reviews day-to-day operations and coordinates management of the practice with the Department Director. Assures compliance with regulatory agencies and the rights of all customers.
KNOWLEDGE, SKILLS AND ABILITY POSITION SUMMARY:
Responsible for reviewing day-to-day clinic operations of physician practices, recommending process improvement, and coordinating management of them with the Department Director. Provide resources to team members, mentors, education, and counsel team members. Responsible for understanding & communicating organizational, hospital, and department goals.
TASKS:
· Day-to-day management of clinic operations, ensuring smooth workflows, providing resources, direction, and support to Providers and Team Members.
· Determines, develops, and implements workflow improvements through coordination with the departmental management team.
· Monitors budget and financial information, including Accounts Receivable, and provides feedback as needed.
· Monitors support staff and provides mentoring, education, and counseling as needed, under the direction of the Department Director and HR.
· Monitors volumes, budget, and financial information and determines opportunities for improvement.
· Assures state and federal and The Joint Commission compliance, including OSHA, CLIA, and JC Survey, and ensures adherence to regulations regarding supervision of clinical and non-clinical staff.
· Monitors, develops and maintains appropriate FTE staff ratios, including the responsibility of payroll and overtime.
· Interfaces with Providers and informs the Department Director of any issues.
· Develop agendas and regularly hold staff meetings.
· Participates in departmental meetings and works as a team with all Departmental Leadership.
· Meets targets set by Corporate, Hospital & Department Director.
· Act as Patient Liasion within the Department and Hospital.
· Other duties as assigned.
Experience Required:
Three years of practice management experience. Multispecialty and/or Primary Care experience preferred. Multi-location management with oversight of more than 8 providers is preferred.
Education: Graduate of high school program.
