Case Manager - Physician Practice coordinates the overall interdisciplinary plan of care for assigned patients in a physician practice setting. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided and that patient is making progress towards treatment goals. Responsibilities
The mid-level provider is responsible for providing professional level primary health care to patients with complex chronic and acute health conditions under the direction of a collaborative provider (if required). They provide general and preventative care, conduct check-ups, treat illnesses, order lab tests and prescribe medication for children and adults (within scope of practice)
