Case Manager (Non-RN) coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Case Manager (Non-RN) typically requires a bachelor's degree. Typically reports to a head of a department/unit. To be a Case Manager (Non-RN) typically requires 2 to 4 years of related experience.     This opportunity is full-time at 36 hours per week, offers full benefits and a night shift schedule.  There is opportunity for overtime and to cover various shifts if interested.

    Job Requirements:

     

    • Reports to the Director of Case Management. Must be a Florida RN.  2-4 years recent acute care experience required.  Experience in utilization chart review and discharge planning.  Managed care experience preferred.  Knowledge of InterQual Criteria.  Strong clinical background needed with strong interpersonal communication skills.  Basic computer skills.

    Qualifications

    Role and Responsibilities

     

    Ensures appropriate utilization of hospital services as well as developing a discharge plan and all appropriate services in timely manner.  Recognizes the signs and symptoms of stroke and calling a stroke alert overhead. Verbalizes stroke alert process. Works with the stroke patient and family for discharge planning and treatment based on needs, condition and prognosis of patient. Maintains currency in stroke knowledge by completing the annual 1 hour online training
