Case Manager (Non-RN) coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Case Manager (Non-RN) typically requires a bachelor's degree. Typically reports to a head of a department/unit. To be a Case Manager (Non-RN) typically requires 2 to 4 years of related experience.     The RN MDS Case Manager is a member of the neighborhood team who works in partnership with direct care nursing staff to ensure that patient care is based on healthcare needs and patient preferences, and adheres to regulatory requirements. The Case Management Nurse's primary role is to complete a comprehensive assessment and care plan for each patient in her/his caseload. S/he also works closely with nursing and social work staff to ensure preparation for discharge.

    Required Qualifications:

    • Washington Registered Nurse License upon hire.
    • 2 years of Nursing experience in LTC, TCU or equivalent experience

    Preferred Qualifications:

    • Bachelor's Degree in Nursing
