Description

The RN MDS Case Manager is a member of the neighborhood team who works in partnership with direct care nursing staff to ensure that patient care is based on healthcare needs and patient preferences, and adheres to regulatory requirements. The Case Management Nurse's primary role is to complete a comprehensive assessment and care plan for each patient in her/his caseload. S/he also works closely with nursing and social work staff to ensure preparation for discharge.

Required Qualifications:

• Washington Registered Nurse License upon hire.
• 2 years of Nursing experience in LTC, TCU or equivalent experience

Preferred Qualifications:

• Bachelor's Degree in Nursing

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses.
