Job Duties:

• Assesses/re-assesses hospice patients.
• Performs nursing services and treatments per physicians orders.
• Communicates with Hospice physician team.
• Acts as case manager to coordinate all aspects of patient care.
• Participates in the on call rotation.
• Supervises hospice aides and other staff members as appropriate.

Qualifications: Must be licensed as a RN in the state of Ohio; must have at least 2 years experience as a nurse, Hospice experience strongly preferred; Leadership qualities are strongly preferred.
