    Your New Role:

    The Alba Case Manager–Crisis Intervention position provides short-term, crisis stabilization case management to 4-6 youth (ages 9-17) accessing emergency housing and shelter services, coordinating with family, school, and other providers, helping to reunify or secure alternate long-term safe housing and other resources. Case Management is provided to East Multnomah County and Portland youth and the responsibilities include intake and assessment; crisis intervention; housing placement and stabilization; individual case management; case planning and coordination; resource brokerage and referral; advocacy and skill building; collaboration with team of family mediation and support specialists as well as community-based case managers; follow-up and after care; provision of clinical and financial and other record keeping related to job duties.

    Knowledge & Skills:

    · Demonstrated knowledge of the issues of substance abuse, mental health, trauma, abuse and adolescent development including brain development.

    · Experience, ability and commitment to working with sexual minority and culturally diverse staff, participants and community.

    · Excellent communication, organization, creativity, flexibility, optimism, positive humor and overall interpersonal skills.

    · Must have access to an automobile, a valid driver’s license, and active auto insurance on that vehicle. Reimbursed for mileage etc.

    · Must be willing to be CPR trained and certified

    Education & Experience:

    · QMHP eligible, Master’s Degree in Counseling, Social Work, or related field OR equivalent education and lived experience

    · Minimum of two years’ experience working with high-risk youth and/or families required, case management experience preferred
