Case Manager - Mental Health Services jobs in York, PA
Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work. Looking for hands-on work that makes a positive impact in people’s lives? This may be the position for you. The successful MH Case Manager will assess individual mental health, drug and alcohol, and/or children’s needs of Possibilities program residents, and make referrals to appropriate services. The Case Manager provides supportive guidance and advocacy for the mental health needs of program participants, including crisis intervention; transport residents as needed to accomplish program goals; and design and instruct or arrange mental health and life skills workshops. Case Manager is responsible for preparing and maintaining required documentation and records, including client-centered goal plans, discharge plans, and aftercare records. This is a full-time salaried position.
Minimum requirements: bachelor’s degree in social science. Must be at least 21 years old, possess PA valid driver’s license, insurable driving record, and have reliable transportation. PA State Police background check, Act 33 and/or FBI clearances. - Just Posted
