Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work.     CareNational is looking for an experienced Behavioral Health Case Manager in Winston Salem, NC! This is a direct hire position and ideal candidates will have at least 2 years of recent experience as a Behavioral Health Case Manager.

    Pay Details:
    • $60,000 - $65,000 is the amount that CareNational reasonably expects to pay someone for this position.

    Work Setting:
    • On Site (Opportunity to go remote after 3 months)

    • Direct Hire
    • Location: Winston Salem, NC
    • Schedule: 8am-5pm EST
    Requirements:
    • Master’s Degree in Social Work, Counseling or related field with a minimum of 2 years’ experience in a community outreach or equivalent position
    • A combination of training and education that meets the above knowledge and skill level with a minimum of 5 years’ experience in a community outreach or equivalent position
    • Demonstrates ability to respect individual/family diversity and maintain confidentiality.
    • Demonstrates ability to work as a team member.
    • Knowledge of and ability to work collaboratively with providers and county/community health and human services.
    • Ability to demonstrate excellent communication skills both oral and written as well as strong interpersonal skills.
    • Proven ability to work independently and to be able to manage time appropriately
    • Strong organizational skills.
    • Computer literate. Must be able to pass computer documentation competency testing for all software platforms used within the program. This must occur within 3 months of initial training and/or 6 months of hire, whichever comes first.
