Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work.     Position Summary:

    Mental Health Case Managers work with individuals in the community to overcome their barriers and obstacles that confront them to recover from their mental illness. They provide a service that includes functions that involve locating, coordinating, and monitoring services and supports for individuals who have a mental illness.

    Day Hours, M-F with on-call (standby rate/overtime, rotating basis).

    Qualifications:

    Bachelor’s degree, which includes or is supplemented by at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social science; OR

    Associate degree which includes or is supplemented by at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice or other related social science AND two years of relevant experience

    Automobile, Valid Driver’s license, PA Child Abuse, PA Criminal History and FBI clearances are required
