Position: Case Manager, Psychiatry Services

Description: The Psychiatry Services Case Manager works as part of the Psychiatry Services team as liaison for clients receiving Psychiatry Services as their only basic care plan while they are transitioning out of or stepping down from a more comprehensive array of the agency’s services. The position supports psychiatric providers and clients and acts as point of contact for the Children’s Psychiatry team and CCPS Psychiatry to facilitate treatment related communication, scheduling, chart management, care coordination, and to implement follow up care instructions to ensure clients are building and maintaining strong ties with other community-based service providers.
