Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. GENERAL SUMMARY:
Under the supervision of the Clinical Care Team Supervisor or designee, the Primary Care Case Manager is responsible for assessing, coordinating and implementing care and services for all patients as assigned. The Primary Care Case Manager develops and implements the patient’s plan of care, coordinating required care by other disciplines and facilitating physician involvement as required.

EDUCATION:
Graduate of an accredited Nursing Program required.
WORK EXPERIENCE:
Minimum of two (2) years experience as a Registered Nurse in a home health setting preferred
Supervisory experience preferred.
SPECIAL EMPLOYMENT REQUIREMENTS:
Must be cleared by PA State Police criminal history check.
Possession of a current license as a Registered Nurse as issued by the Pennsylvania State Board of Nurse Examiners required.
Must be knowledgeable of Conditions of Participation for Home Health Agencies and HIM-11 regulations.
Excellent written and verbal communication skills, effective listening skills and interpersonal skills required for effective communication with staff, employees, and visitors; for working effectively with others; to motivate staff; and to elicit work output.
Must have strong clinical skills, organizational/management skills, and ability to remain objective.
Ability to provide leadership and creativity in the management and development of staff in an environment of change; and to plan, organize, and coordinate staff function’s work flow required.
Must have ability to integrate clinical and financial data.
Valid PA driver’s license and available transportation during all working hours required.
PHYSICAL DEMANDS:
Moderate Work: duties require exerting up to 25 lbs. of force occasionally (approx 10-33% of shift), and/or up to 10lbs. of force frequently (approx 33-66% of the time), and/or a negligible amount of force constantly to lift, carry, push, pull, and move objects (i.e., occasional patient transfers).
Position involves bending, reaching, stooping, and lifting (occasionally in strained positions) throughout shift.
Visual/hearing ability sufficient to comprehend written/verbal communication required.
Manual dexterity sufficient to perform duties such as typing and computer work required.
Must be able to travel as required.
Ability to transport self throughout facility is required.
ACCIDENT OR HEALTH HAZARDS:
Exposure to blood and body substances.
Exposure to needle sticks possible.
WORKING CONDITIONS:
Works in a well-lit office area and in varying conditions in patient homes (where conditions may be less than desirable).
