Job Summary: Under the supervision of the Program Director, the Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. In this capacity, the Service Coordinator (Mental Health Case Manager) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.
