About the Organization and Position

A wonderful, well-established non-profit in San Francisco is searching for a Therapist / Clinician for their behavioral health program. This organization is at the leading edge of practical and innovative solutions to serve the marginalized populations of San Francisco. Their core services include homeless wrap around services, substance abuse and behavioral health including co-occuring disorders. This well respected organization has been a staple in the community for over 50 years.

The expectation of a Housing Case Manager services to provide exceptional services to our guests; conduct intake, match, and refer guests to housing services; assist guests with completing housing application paperwork; help guests complete surveys/questionnaires. As a Housing Case Manager, you will connect guests to appropriate resources, including existing behavioral health/clinical services, financial coaches, workforce development counselors, medical, SUD, and/or mental health, education, and skill-building services.

Responsibilities

• Complete case management assessment on all clients entering caseload, develops, implements and revises individual case plans and consistently monitors progress toward goals.
• Provide ongoing supportive and/or case management functions in accordance with the problems, needs, and strategies identified within the case plan to help the clients achieve the stated goals and objectives. This includes communicating regularly with probation officers, social workers, and family members, regarding the client’s treatment progress, etc., when appropriate.
• Works with clinical supervisory assistance to evaluate possible courses decisions regarding client status and makes decisions where there is no opportunity to seek supervisory assistance.

Requirements

• Bachelors in Behavioral Health, counseling, or related field, preferred
• Excellent communication and interpersonal skills; proven ability to collaborate with other agencies.
• Experience working with substance use disorders, mentally ill, and dually diagnosed
• clients highly desirable.
• Clinical supervision toward licensure is available for those on a BBS licensure track if selected.
• Fluency in English and Spanish is preferred.
