SUMMARY: The Case Manager IV will provide case management to shelter clients and assist them in achieving greater self-determination, self-sufficiency and permanent housing. The case manager IV will provide additional support to the other case managers and review how cases are being handled to better utilize resources. The CM IV will operate in a supervisory position in the absence of the PM to offer support and guidance to the staff.
ESSENTIAL DUTIES AND RESPONSIBILITIES: (These essential job functions are not to be construed as a complete statement of all duties performed. - Set attainable goals and monitor progress of clients, find the barriers and ways to remove them to attain self sufficiency
- Design of Housing Plans specific to barriers and additional needed support for clients
- Assist clients with varying needs (i.e. general information, housing, apartment searching, veteran assistance, and referrals)
- Follow up and evaluation on progress of clients then report to management on client progression-adjust the plan to meet the specific barriers on a client-by-client basis
- Meet to monitor progress and maintain accurate documentation of progress towards goals and services provided
- Knowledge of harm reduction, motivational interviewing and trauma-informed care.
- Maintain accurate required documentation, including entering required client information into Pinellas Homeless Information Network (PHMIS), as required
- Post information relative to job skills, housing, social events on the bulletin board-provide different resources to meet the clients need
- Organize and conduct client satisfaction surveys and perform other duties relative to the position including typing, filing, answering telephones, etc.
- Assist with client access to services and community resources as needed
- Promote an environment of teamwork and collaboration
- Take the lead on decision-making in the absence of the PM.

- Advanced knowledge of homelessness, severe and persistent mental illness and substance abuse
- Advanced knowledge of resources in the community available for veteran population, especially services and programs offered by the VA

- Must be able to comply with complex governmental regulations, policies and procedures and demonstrate thorough document compliance efforts and activities
- Must be proficient with data management and information systems and have basic knowledge of Excel, PowerPoint, and Outlook

EDUCATION AND EXPERIENCE: (Pending on position and if prior to hire is approved by Chief Executive Officer, a comparable amount of training, education or experience may be substituted for the minimum education qualifications)
- PHD and/or experience 15-years’ experience (work, life or volunteer) in social services.

GENERAL PHYSICAL DEMANDS: These physical demands are representative of the physical requirements necessary for an employee to successfully perform the essential functions of the job. WORK ENVIRONMENT: Environment will occasionally become noisy due to equipment operations and interactions among clients and staff. There may be the possibility of being exposed to communicable disease, possible exposure to verbal abuse or similar behavior from residents/clients. On an as needed basis, employees may be called upon to work outside of the established work schedule or work odd hours. All information associated with the Agency is confidential.
