Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. The Sylvia Brafman Mental Health Center is seeking a Case Manager to fulfill our position available in Tamarac, FL. As a Case manager, you will facilitate our clients’ care by assessing their needs, evaluate treatment options, plans, and be involved in evaluating various services to help our clients gain function and independence. Case Manager will help clients connect with resources that can help them with their challenges, whether they be mental health or substance abuse. The ideal candidate will have knowledge of mental health disorders, treatments, and community resources. Bilingual preferred but not required.
