Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work.   • Qualifications
    Individual must possess a Master’s Degree (MA) from an accredited college or university in social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice; or have a minimum of two years of professional experience in a community behavioral health setting. Certification as a psychiatric rehabilitation practitioner may be substituted for one year of experience. Valid driver’s license in good standing.

    Responsibilities

    • Provides community based outreach services to persons diagnosed with a psychiatric disorder.
    • Performs case management duties including linkage and referrals to mental health and non-mental health services, service coordination and assists with therapeutic rehabilitative skill development.
    • Contributes to the development of individualized treatment plans.
    • Services will be delivered in individual’s home or other locations convenient for the individual. Participates in team and community provider meetings.
    • Electronic Clinical Record keeping will be required as indicated by funding sources and/or accreditation agencies.
    • Case managers must meet all expected productivity levels for direct service provision to consumers.
