Responsibilities:
- Conduct comprehensive assessments of clients' needs, including social, emotional, and financial factors
- Develop individualized care plans in collaboration with clients, outlining goals and objectives
- Connect clients with appropriate community resources, such as childcare services, Section 8 housing, behavioral health providers, and workforce development programs
- Provide ongoing case management services, including regular check-ins and monitoring progress towards goals
- Advocate for clients' rights and ensure they have access to necessary services and supports
- Collaborate with other professionals involved in the client's care, such as healthcare providers, educators, and social workers
- Provide crisis intervention as needed to ensure the safety and well-being of clients
- Maintain accurate and up-to-date documentation of all client interactions and progress
Qualifications:
- Bachelor's degree in social work or a related field (Master's degree preferred)
- Previous experience in case management or a related field is highly desirable
- Knowledge of child welfare systems, Medicare/Medicaid programs, and crisis intervention techniques
- Strong communication skills, both written and verbal
- Ability to work independently as well as part of a team
- Excellent organizational skills with the ability to prioritize tasks effectively
- Proficiency in using computer systems for documentation purposes
