Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work.     StarCare Specialty Health System is seeking qualified, compassionate  Adult Mental Health Case Managers to join the Adult Mental Health Outpatient Team!

    Work is performed under the supervision of the Director of Adult Mental Health Services with moderate latitude for the use of initiative and independent judgment.

    Adult Mental Health Case Managers Responsibilities:

    • Providing hands-on, in vivo skills training, symptom management, support, and crisis intervention;
    • Addressing housing, money management, employment issues, and living arrangements;
    • Communicating pertinent clinical information;
    • Providing face-to-face contact with clients and their support systems;
    • Evaluating, monitoring, linking and coordinating services for clients on assigned caseload;
    • Ensuring clients are aware of resources necessary to reach personal outcomes;
    • Ensuring agency resources are utilized appropriately; and
    • Completing and submitting required reports and related documentation.
    • This position ensures the all activities support the vision, mission and values of StarCare Specialty Health System.

    Clinical supervision is provided by the Mental Health Division's Licensed Professional of the Healing Arts (LPHA).

     

    Schedule: This is a full-time/non-exempt position scheduled Monday - Friday, 8am - 5pm, with flexibility to meet the needs of the department. 

    Incentive Program

    Upon Completion of First 30 Days       $500
    Six (6) Month Anniversary $500
    One (1) Year Anniversary $1,000
    Two (2) Year Anniversary $2,000

    Qualifications


    Qualifications:

    • Bachelor’s degree from an accredited college or university with a major in the Human Services field (Psychology, Social Work, Human Development/Family Studies, etc.), required.
    • Two (2) years of full-time related work experience, preferred.

    Additional Requirements:

    • Knowledge and skill in operating standard office equipment and personal computer with Microsoft office products is required.
    • Local travel using personal vehicle is required (mileage reimbursement provided at state approved rate).
    • Must have reliable transportation, current Texas driver’s license, and be insurable under agency insurance coverage.
    • Agency provides a cell phone and laptop computer.
    • Must pass a pre-hire drug screen and criminal background check.
