Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work.     Stonington Institute is committed to providing quality substance abuse treatment options for adults in Southeastern Connecticut and surrounding areas. We offer medical detox as well as residential addictions treatment for active duty and retired military service members in our North Stonington location. Our clinic in Groton, CT, offers partial hospitalization and intensive outpatient treatment, and is conveniently located across the street from The Inn at Trails Corner, a new 180 bed dormitory-style sober community for adult men and women who attend our outpatient day treatment programs. The Inn offers overnight accommodations, full-meal service, indoor/outdoor recreation options, a workout room, and computer center for all clients. We accept private insurance, state-funded plans and self-pay arrangements.

    Job Summary:

    The Director of Case Management Services (DCMS) supervises the case management staff and oversees the discharge and aftercare plans for the Adult Partial Hospital and Intensive Outpatient programs (PHP/IOP). S/He is responsible for ensuring that a high quality, cost-effective case management service is delivered by the department, in keeping with the program's goals and objectives. The DCM ensures that the case managers provides direct client care specific to each client's needs and assures each client is at the appropriate level of care. The DCM is involved in the assessment, treatment planning and service coordination process for all cases. This individual serves as a key member of PHP/IOP leadership.

    Qualifications

    Bachelor's Degree required; Master's degree in healthcare-related field preferred. Five (5) years of case management experience, plus 3 years of experience in a management position required. Experience must be in a residential or outpatient substance abuse/behavioral health setting. Demonstrated leadership within the agency may be substituted for supervisory experience.
