Case Manager - Mental Health Services jobs in Miami, FL

We are currently seeking experience Targeted Case Managers, to work around the area of Miami Dade County. The TCMs will be assigned pre-release and/or post-release clients and will complete the assessments, develop case management plans, coordinate referrals, ensure access to needed services, and explain the need for services to clients/providers. Serve as liaison between client and family, and community agencies. Provide case management functions of assessing, planning, linking, monitoring, and advocating for individuals. Assume primary responsibility for developing, writing, implementing, evaluating, and revising overall case management goals.

Must hold a BSCHM Certification of Target Case Manager or Grandfather Certification by the Florida Certification Board, Bachelor’s degree from an accredited college or university in Psychology, Social Work, Mental Health, or related field, a minimum of one (1) year of experience as a Target Case Manager with adults' emotional-behavioral problems is required. * Must be* _ Bilingual – English and Spanish_. Computer literate (Proper use of Office Windows Words, Excel, PowerPoint, outlook, Lauris, or Cedrus Med.

Requirements:

• Bilingual in English and Spanish fluently (Must speak both).
• 50 hrs Target Case Management Training Certification. (Must).
• Must hold a CBHCM certification by Florida Certification Board. (Must)
• First Aid/ CPR Certificate (Must be obtained within the 90 days probation period).
• FARS and CFARS (Must be obtained within the 90 days period).
• Minkoff (Must be obtained within the 90 days period).
• NPI (Must obtain NPI within the 90 days period).
• Willing to undergo clearinghouse AHCA level 2 fingerprinting. (Required as part of the hiring process).
• A local Police background check (Required as part of the hiring process).
• Physical and TB test (Required as part of the hiring process).
• Willing to comply with all requirements in a timely manner.
