Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work.     About the Program:
    Our office is known for its strong commitment to taking care of those in need in our local community. We provide persons in need of mental health support with wraparound services which include counseling, therapy, EMDR, medication management and employment services. Our client population includes chronically homeless individuals and/or those with persistent and serve mental illness.

    Our staff is comprised of a tightly knit team of local therapists, case managers and psychiatric prescribers who enjoy a laid back, casual work environment. Candidates can expect a caseload of around 50 clients, providing mental health counseling and case management
    Duties & Responsibilities:

    • Provide ongoing coordination and collaboration of case management services
    • Provide treatment planning, resource identification and development, and advocacy
    • Provide service monitoring and follow-up

    Requirements: (Any Equivalent Combination of Knowledge, Skills, Abilities, Education, and Experience)

    • Education: Bachelors in Psychology, Social Services, or related field.
    • License: Agency Affiliated Counselors license or the ability to obtain one.
    • Experience: Supervised practical experience in a clinical or rehabilitation (recovery) setting with persons with severe and persistent mental illnesses.

    Schedule:

    • M – F, 8:30am – 5pm

    Compensation:

    • $22.05 - $23.54/hour
    • Wages are determined by applicable industry experience, education, and certifications/licensure.
