Case Manager - Mental Health Services jobs in Maine
Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work. Description
AMHC has an immediate opening for a Full-Time Adult Behavioral Health Home Coordinator-Community Integration Specialist (BHH Coordinator) in our Ellsworth area. The Adult BHH Coordinator is responsible for assisting clients with long-term psychiatric disabilities to attain the skills and resources necessary to successfully function in the community. The person will emphasize is placed on holistic care using 8 dimensions of wellness: Emotional, Financial, Social, Spiritual, Occupational, Physical, Intellectual, and Environmental.
Qualifications:
- Minimum of an associates Degree in a Human Services related field is required
- MHRT-C certification or MHRT-C Provisional certification is required.
- Personal vehicle use-required.
Responsibilities & Expectations:
- Establish Compassionate therapeutic relationships with clients and prospective clients demonstrating the Integrity of AMHC
- Perform Collaborative case management functions, linking, coordinating, and managing the delivery of services to clients with Excellence
- Provide Respectful, supportive counseling and assisting clients in selecting and weighing alternatives and making a decision with a focus on Service for the client.
- Coordinate client's physical and mental health needs
Schedule:
- This is a 40-Hour per week position
