We are seeking a dedicated and compassionate Intensive Home Based Case Manager to join our team. As an IHBT Case Manager, you will be responsible for providing comprehensive care coordination and support to youth & their families in need. This role requires strong communication skills, attention to detail, and the ability to work collaboratively with a multidisciplinary team.
Responsibilities:
- Conduct client assessments to determine individual needs and develop personalized care plans
- Coordinate and facilitate communication between patients, healthcare providers, and other stakeholders
- Advocate for clients' rights and ensure they receive appropriate services and resources
- Monitor client progress and make necessary adjustments to care plans as needed
- Provide mental health and addiction case management as required
- Educate clients and their families on medical conditions, treatment options, and available resources
- Maintain accurate and up-to-date documentation of patient interactions and progress
- Adhere to HIPAA regulations regarding patient privacy and confidentiality
- Collaborate with social workers, children services, schools, probation officers & other professionals to ensure coordinated care
Qualifications:
- Bachelor's degree in social work or related field
- Previous experience in case management or related healthcare field
- Knowledge of youth & family services
- Familiarity with mental health & addiction case management techniques
- Excellent communication skills, both written and verbal
- Ability to work independently as well as part of a team
- Strong organizational skills with attention to detail
