Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. We are a certified outpatient mental health office licensed with the New Jersey Department of Human Services doing individual and group therapy work to help clients from all of NJ.
Responsibilities:
- Provide clinical supervision and oversight to a team of LSW's & LAC's
- Conduct regular individual and group supervision sessions to support professional development and ensure quality of care.
- Review and approve treatment plans, progress notes, and other medical documentation.
- Monitor client progress and outcomes through data collection and analysis.
- Collaborate with other members of the clinical team to develop and implement evidence-based treatment protocols.
- Provide direct psychotherapy services to clients as needed.
-Advocate for clients and lead in conflict resolution and crisis management.
- Conduct assessments and evaluations to determine appropriate treatment interventions.
- Provide crisis intervention and support as necessary.
