Essential Duties/Responsibilities
The Case Manager is responsible for providing psychiatric support and rehabilitation services that are focused on improving the client’s ability to succeed in the community, home, work and school. These include but are not limited to the following:
- routinely assess the needs of the client
- assist client in achieving personal independence in managing basic needs
- Facilitate in the development of daily living skills
- Symptom monitoring
- Coordination and assistance in crisis management and stabilization
- Advocacy and outreach
- Education and training specific to the needs of the client and family
- Interventions that eliminate barriers to seeking or maintaining education and employment
- Activities that increase the client’s capacity to positively impact his/her environment
- Interact routinely with patients and care givers in the home, school and community to assist client in resolving mental health issues that interfere in daily functions behaviors and communicate significant observations to clinical staff.
- Assist with implementing interventions to meet treatment goals.
- Document client progress and interventions
- Document information regarding patient care and behavior as required in policy and procedure.
- Run errands related to patient care and therapeutic needs.
- Supervise patients in therapeutic care as assigned.
- Utilize Crisis Prevention Intervention when exposed to volatile patients to maintain personal safety while ensuring safety for the patient and other individuals in the office.
- Participate in patient treatment planning and implementation under the direction of Clinical Director and other professional staff involved in patient care.
- Participate in maintenance of a clean, orderly and safe work/therapeutic environment; report needs for repairs and maintenance to appropriate staff.
- Assist in admissions and discharges of patients as assigned to include entry of patient information in EHR system.
- Work under the direction of the Clinical Director to ensure all duties are completed and address training needs.
- Conduct and assist with selected activity groups as assigned and document according to policy and procedures.
- Attend and participate in office in-services and staff meetings.
- Assist with assessments as assigned.
- Participate in implementation of therapeutic programming. Assist in planning and leading therapeutic groups.
- Ability to demonstrate competent use of Electronic Health Records
- .Maintain confidentiality in accordance with established HIPPA and office policy, procedures and standards of care.
- Performs other duties as assigned.
Minimum Qualifications/Requirements
Earned Bachelor's degree in Psychology, Social Work, Sociology or Counseling.
Certificates, Licenses, Registrations
CPR/First Aid/AED certification.

- Exposure to disturbed and potentially aggressive patients is possible. Stress ranges from low to intense depending on situation.
- Sitting, standing, walking, reaching, bending, stooping as required to assist in patient care.
Conditions of Employment
- Completion of a pre-employment drug screening and completion of post-employment drug or alcohol tests upon reasonable suspicion of use.
- Completion of Center-wide orientation and ALL required paperwork prior to reporting for work.
- Demonstrated computer literacy through successful completion of pre-employment testing may be required.
- Completion of State mandated, and office required training,
- Completion of AED Training, Completion of Crisis Prevention Intervention (CPI) Training, Completion of CPR/First Aid Training.
- Attendance at all mandatory staff development and training.
- Successful completion of a six month on the job orientation period.
- Participation in payroll electronic deposit.
- Adherence to Compliance Program Plan.
- Satisfactory reference and background investigation checks.
- Must possess a Bachelor's or Master's degree in Psychology, Social Work, Counseling, Sociology, Early Childhood Education or Rehabilitation Services. No other degrees will be accepted.
