Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work.     What your role entails:
    • Evaluating mental health diagnoses and implementing treatment plans.
    • Maintaining updated documentation using company platforms and EHR.
    • A minimum of 25 client sessions per week to a diverse population of clients (with the ability to see more clients if desired)!
    • Provide excellent customer service for clients and collaborate with a dynamic team to further the mission of filling gaps in our community
    • Utilize creativity in interventions to help clients achieve and exceed goals
    • Care coordination for assigned clients to ensure continuity of care.
    • Participation in clinical staff meetings, trainings, supervision, and community outreach.
    • Community involvement and engagement.
    • A sense of humor and desire to have fun in the workplace will certainly help as well!

    Required Qualifications
    • Candidates are required to have a master’s degree in one of the behavioral sciences or related fields from an accredited college or university
    • Candidates should have clinical licensure in the state of Connecticut (LCSW, LPC, LMFT, PsyD, PhD) in good standing
    • Experience with completing intake assessments, treatment plans, and clinical case notes
    • Effective communication skills
    • Ability to demonstrate and model stable, appropriate boundaries with clients
    • Ability to complete and submit documentation of services in a timely manner
    • Comfort and familiarity working with a diverse client base.
    • Ideal candidates will have a general knowledge of community resources and insurance billing
    • General knowledge of Apple products, EHR, Telehealth, and other software platforms
    • Ability to pass a background check.
