Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Our comprehensive behavioral health center located in Grand Rapids, Michigan offers psychiatric urgent care, outpatient, inpatient, partial hospitalization (day programs), assessment and testing, residential, addiction treatment and specialty services such as crisis response, employee assistance programs, forensic psychiatry and psychology, neuromodulation and more.
Under general supervision and according to departmental policies and procedures and within the individual plan of care performs the role of physical and emotional care given to clients with mental illness and/or developmental delays. Is responsible to actively participate in safety management, milieu management, crisis intervention and prevention. The Psychiatric Technician must demonstrate excellent customer service and have enthusiasm about the program and the Pine Rest organization.
