    EMBRACE’s Clinical Case Management Program partners with school districts to find residential programs and provide clinical oversight while students reside in placement.

    As a Clinical Case Manager (CCM), you will support the school district in determining which residential treatment centers (RTCs) meet a student’s unique educational and mental health needs. You will also manage the referral process and collaborate with the school district as they make appropriate placement offers. The CCM conducts quarterly visits (both in and out-of-state) with the student and RTC team to monitor treatment, milieu, and educational goal progress, while also providing clinical support to caregivers throughout treatment. This clinical oversight, on behalf of the school district, helps to ensure appropriate fit, reasonable length of stay, and a timelier return home for students.

    Responsibilities include:

    • Determine prescriptive placements that meet the educational and mental health needs of students
    • Manage the RTC referral process and offer clinical recommendations to school districts
    • Visit students at RTCs on a quarterly basis to monitor treatment, environment, and educational goals and concerns
    • Facilitate communication between caregivers, RTC staff, and school districts
    • Participate in IEPs and monthly treatment team meetings
    • Provide clinical consultation and case management assistance throughout a student’s stay in residential treatment
    • Complete documentation in a timely manner

    Qualifications:

    • Candidates must have a graduate degree in psychology, social work, or a related subject and be licensed (LCSW/LMFT/LPCC) or a registered associate with the BBS (ACSW/AMFT/APCC).
    • Have a total of at least 2 years of experience providing clinical services to a culturally, ethnically, and socioeconomically diverse clientele, preferably in community or school-based settings.
    • Must have a dependable vehicle and good driving record.
    • Must pass all required background checks and TB test clearance
    • Be able to communicate effectively, verbally and in writing, displaying active listening and collaboration skills.
    • Be able to demonstrate resourceful problem-solving skills and flexibility in a dynamic work environment.
    • Be detail-oriented with efficient organization strategies.
    • Be able to observe and meet critical deadlines.

    Schedule and Location

    • Full-time and part-time opportunities available, normal business hours Monday through Friday in Pleasant Hill office.
    • In and out-of-state traveling required, which may extend past normal business hours.
    • Clinicians see clients in their homes, schools, the EMBRACE clinic and via Zoom.
