Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Under the direct supervision of the Chief of Psychosocial Rehabilitation Program, provides ongoing case management services for children/adolescents and adults with mental illness.
Essential functions and responsibilities:·
Specific Equipment and Materials to be used: Personal vehicles may be used for consumer visits; use of company vehicles when possible. Personal laptops and phone may be used as well. Gas stipend provided.
Education: Bachelor's degree from an accredited college or university in Behavioral Health field, which includes human services/or behavioral sciences related (psychology, sociology, social work) field from an accredited college or university.
Experience and Training: 0-3 months of successful case management experience in Behavioral Health Services. Prefer bilingual English/Spanish. Requires a valid driver license with a safe driving record along with proof of a current and valid auto insurance card.
Physical activities/Physical demands: Sitting 2/3 of time. Stand, walk, bend, stoop, and kneel, reach with hands and arms, twist at the waist, push and/or pull less than 1/3 of time. Lift and carry maximum of 35 pounds
