    Duties:

    - Conduct comprehensive assessments of patients' medical conditions and develop individualized care plans

    - Collaborate with healthcare providers to ensure effective communication and coordination of patient care

    - Monitor patients' progress and make necessary adjustments to the care plan as needed

    - Provide education and support to patients and their families regarding their medical conditions and treatment options

    - Coordinate discharge planning and ensure smooth transitions between healthcare settings

    - Utilize critical thinking skills to identify potential issues or barriers to patient care and develop appropriate interventions

    - Maintain accurate and up-to-date medical records

    - Collaborate with interdisciplinary teams to ensure optimal patient outcomes

    Experience:

    - Valid Registered Nurse (RN) license

    - Minimum of 2 years of experience in a clinical setting, preferably in pediatrics or critical care

    - Strong knowledge of medical terminology, disease processes, and treatment modalities

    - Experience with utilization management and managed care principles

    - Excellent communication and interpersonal skills

    - Ability to work independently and as part of a team

    - Proficient in computer skills, including electronic medical record systems
