Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work.     Linn County Mental Health is seeking an experienced Qualified Mental Health Associate to work with individuals with severe mental illness.. Duties will include working in the community and in client homes, providing case management, facilitating groups, providing skills training, and other supports that help clients be successful in the community. Other areas involved with this position include Strong communication skills; computer skills, experience using electronic medical record; a background with mental health documentation; understanding of basic counseling and assessment, evidence-based practices, , outreach and crisis interventions; and writing and supporting treatment goals. The successful applicant will work closely with colleagues as part of an interdisciplinary team and should possess the ability to be flexible with case and task assignments. Timely completion of paperwork and documentation is critical. This position may require evening and weekend work.Graduation from a four-year college or university with major coursework in psychology, social work, counseling, or closely related behavior science field; or four years of progressive experience in a mental health service program involving general client needs; or any satisfactory equivalent combination of experience, education and training which demonstrates the ability to perform the work described. Applicants must register with The Mental Health and Addiction Certification Board of Oregon (MHACBO) within 30 days of employment.
