General Description
PATH caseworker will serve as a liaison between homeless individuals and mainstream community service system. Services will be mobile in nature contacting homeless persons in the community to identify service needs to reduce homelessness by linkage to appropriate agencies within the community. Ongoing services will be provided to the homeless population identified as having a mental illness. Understands and implements trauma informed care practices. Responsible for delivery of case management. Identifies clients' rehabilitation needs and provides the necessary instruction in teaching self-help, self-care, or other skills according to the needs outline in client's recovery plan. Crisis prevention and intervention will be carried out. Services will be provided in the community with the goal to successfully mainstream this population into community services. Individual improvement of homeless individuals is based on the following functioning scales: decrease the numbers of incarcerations; stability of mental health functioning; decrease housing instability; decrease of substance use; and engage individuals and link to services to improve perception of recovery. Works under PATH supervisor with limited supervision and considerable latitude for the use of initiative and independent judgment.
Minimum Qualifications
Bachelor's or advance degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education or early childhood intervention. Knowledge of community resources of case management principles, objectives, standards, and methods of program policies and procedures. Ability to assess client needs, coordinate client services and communicate effectively. Must have a Valid Texas Driver's license with a good driving record and reliable transportation. Preferred: One year of work experience in social services. Bilingual in English and Spanish.
