· Hours will increase each week based on how quickly you can schedule your clients.

Education:
· Bachelor’s degree in social work, counseling, psychology or a related health care field, from an accredited institution
Training & Experience:
· At least 3 years background in mental health targeted case management working with adults and seniors preferred.
General Description of Duties:
· Provides behavioral health services to children with mental illness. Adheres to the spirit of the mission statement while performing assigned duties.
· Position may require to work outside of a 9am-5pm schedule based on client’s schedules.
Provide case management services (information, plan referral and linkages) to Medicaid children and families.
Document and track outcome measures through TCM assessments, TCM service plans, regular documentation through TCM notes, and complete weekly billing/timesheets.
Comprehensive assessment/periodic re-assessment of the individual’s need for medical, educational, social or other services.
Activities/Interventions to gather/confirm information coming from the Individual, support network and other sources in order to complete assessment.
Determining with the individual and their supports their ability to access and effectively link to these services and supports on their own and what type of help they will need, including how intensely and for how long case management services will be needed.
Prioritizing with the Individual and their Family/Supports the referrals and linkages needed so the treatment/service plan reflects the case management assessment.
Supervision Received:
· Works under the direct supervision of the Community Services Team Lead.
Knowledge, Skills, and Abilities:
· Fluency in Spanish and English preferred.
· Excellent working knowledge of behavioral treatment for children and adolescent behavioral health.
· Ability to work through assessments of behavioral health conditions.
· Ability to collaborate and work as a team with therapists and joint staff in the treatment of psycho-social issues.
· Sensitivity to low income and ethnic minority community a must.
· Experience working with Medicaid populations.
Technical skills:
· Must know how to work on Excel, Microsoft word, and Adobe to complete required paperwork.
